In the US at least, some employers offer health insurance and some don’t. For those who do, it’s very unusual if they pay all of it, almost all companies would have some employee payment for the insurance.

usually for a group health plan the employer pays at least 50% and at least 70% of the employees are enrolled. you must be a w2 employee and work at least 25 hours a week…depending on the insurer ofcourse. it varies slightly
its completely up to the employer (as of now) weather or not they want to offer you benefits
